Bringing care closer to home

Across Victoria, AV paramedics are helping bring preventative healthcare closer to communities that may otherwise struggle to access local services.

Sixteen Paramedic Community Support Coordinators (PCSCs) work alongside healthcare providers, community organisations and local councils to improve health outcomes through education, early intervention and local partnerships. Drawing on their frontline clinical experience, they help identify community needs, strengthen local emergency response capability and connect people with the care and services they need.

These conversations are about more than a blood pressure reading or heart health check. They're an opportunity to empower people with the knowledge and confidence to take charge of their health, understand when and where to seek care, and connect with the support available in their local community.

Community members receive practical health information, advice about local healthcare services and guidance on accessing further care when needed. Sessions also promote important health messages such as recognising the signs of stroke, staying safe during extreme heat and choosing the right care pathway for their needs.

By working closely with local healthcare providers, our PCSC’s can help identify health concerns early and support timely referrals to appropriate services, improving health outcomes and strengthening connections within the communities we serve.
